Pharmacists central to medication safety

PHARMACISTS are uniquely
trained to be able to impact
medication safety at the individual
patient level through medication
management skills that are part
of the clinical pharmacist’s role,
according to a new report in the
Drugs and Aging journal.
The article identifies the
elderly as most likely to benefit
from pharmacist’s monitoring,
analysis and medication safety
programs because they are most
at risk of polypharmacy and drug
interactions.
“Errors may occur in
the storage, prescribing,
transcription,preparation and
dispensing, or administration and
monitoring of medicines,” the
article says, with pharmacists well
placed to help redesign processes
to improve safety.
