PFIZER says the mix-up of heparin and lignocaine packaging which led to a recall earlier this month (PD 14 Aug) did not take place within its manufacturing, packaging or distribution facilities.
A Sydney pharmacy dispensed the medication to a dialysis patient but staff at Concord Hospital later realised rather than heparin, it in fact contained lignocaine.
A spokesperson for the company said "Pfizer is confident this is an isolated incident involving one pharmacy after the products left Pfizer's distribution chain".
The affected batches are still the subject of recall as a precautionary measure.
