Hospital pharmacists in
Massachusetts General Hospital,
Boston have been able to
demonstrate the value of pre- and
post-discharge medication reviews.
Pharmacists who contacted
patients within 72 hours of
discharge discovered that more
than half of the patients had
medication-related issues, Drug
Topics reported.
A separate evaluation period
showed that 35% of patients had
medication issues when reviewed
by a pharmacist prior to discharge.
The research was headed by
Laura Carr, PharmD who presented
findings from the hospital’s
STAAR (State Action on Avoidable
Rehospitalizations) initiative, based
on data collected between July
