Home-based MTM

A PHARMACIST-LED, home-based medication therapy management (MTM) plan for senior patients with diabetes and hypertension has proved a success in Houston, Texas. Led by Texas Southern University College of Pharmacy and Health Sciences, pharmacists visited the patients in their homes to provide them with education and information. During the four month project patients experienced modest reductions in systolic blood pressure and medication adherence scores improved. Focus groups held with participants at the end of the program revealed they were “extremely satisfied” with the program and changed their health behaviours based on the knowledge they gained.
